   Degeneration of joint cartilage and the underlying bone, most common from middle age onward. It causes pain and stiffness, especially in the hip, knee, and thumb joints. 
   Osteoarthritis is an inflammation of the joints, involving the breakdown of the cartilage at the ends of bones. 

In simplest terms, osteoarthritis is nothing but a failure of the joint pattern. It is certainly a degenerative joint disease, characterized by degeneration of articular cartilage and proliferation of new bone, cartilage and connective tissue at the same time.Often secondary inflammatory changes are observed in the synovium. 
 Sadly, the incidence of osteoarthritis seems to be on the rise according to global surveys. The age onset of osteoarthritis is reducing drastically over the period of last few years. Now the incidence is slated to start at joints. Older women are peculiarly affected more and certain geological variations are found too. 

Pathology of osteoarthritis

 Osteoarthritis is a disease of both articular cartilage and subchondral bone. The disease is characterized by progressive degradation of the components of the extracellular matrix of the articular cartilage associated with secondary inflammation factors and increased none turnover and repair.  

Causative and risk factors for Osteoarthritis: 

The primary etiology when the Osteoarthritis is due to aging process it is known as primary osteoarthritis when there are other conditions in the background that have triggered this disorder, it is known as secondary osteoarthritis. 

Secondary Osteoarthritis

-Injury: Can damage to bone, ligament, and cartilage that ultimately leads to severe pain. 
-Infection: Any kind of infection of the joint may result Osteoarthritis. 
-Obesity and advanced age: Both are the very common cause of Osteoarthritis. 
-Overuse: Overuse of knee joint can cause bursitis which ultimately leads to intense pain. 
-Sprain: Sudden unnatural movements causes pain as well as restriction of movement of the joint. 
- Intra-articular fracture
- Occupational (elbow in pneumatic operators)
- Rheumatoid arthritis
-Tabes dorsalis
-Diabetes mellitus
-Peripheral nerve lesions
-Metabolic diseases like Wilson's disease
-Hip dysplasia 
-Paget's Disease
-Malignancy: Myeloma, metastatic carcinoma.
-Dislocation: Also causes severe carcinoma. 
-Other causes: Include sarcoidosis, sickle cell disease, lupus,   Kawasaki disease, Crohn's disease, bone tumors, bleeding disorders. 
- Sedentary habits, improper diet habits, sports injuries, absolute lack of exercise or over exercise regimes can be called as some of the modifiable risk factors for Osteoarthritis. 

Complications

  Osteoarthritis is a degenerative disease that worsens over time. Joint pain and stiffness may become severe enough to make daily tasks difficult. Some people are no longer able to work. When joint pain is this severe, doctors may suggest joint replacement surgery. 

Affected areas

 Osteoarthritis can damage any joint in your body, the disorder most commonly affects joints in your hands, neck, lower back, knees, and hips. Symptoms may include joint pain, tenderness, stiffness, locking and sometimes an effusion. 
- Hands: limitation of activity that requires dexterity are affected.
- Hip disease: The first sign is usually pain or restriction of internal rotation followed by pain on flexion. 
- Knee: Tibiofemoral arthritis is usually bilateral and associated with early morning stiffness and pain on weight bearing particularly on stairs. Night pain is a sign of severe disease. In elderly patients, knee effusions and crepitus are common. 

Signs and symptoms of Osteoarthritis

Characteristically the first few joints to be involved are distal and proximal interphalangeal joints, carpometacarpal joints of the thumb. 
- Gradual onset of symptoms in most cases, joints of spine, hands, and hips are involved. 
- At least initially, only one or a few joints are affected. 
- Knee and hand involvement is peculiar. 
- Intermittent pain and aching in joints typically are aggravated by movement of that joint and is better by resting it. 
- With the advancement of the disease process, limited movement of joints is noticed. Initially, this restriction of movement is attributed to pain and spasm of muscles but later capsular fibrosis, osteophyte formation, and remodeling of bone set in. 
- After minor sprains or twists, there are usually effusions into the joints when crepitus may be felt or even heard. 
- Associated muscular wasting is noted, because of which there is the loss of muscle control over the joint, resulting in recurrent injury. 
- Due to hyperemia of subchondral bone, there is usually nocturnal aching in bones.
- Deformities like Heberden's or Bouchard's node formation, and finally Osteoarthritis deforming may set in.

Bryonia Alba

  The excellent remedy which is useful in the treatment of  Osteoarthritis, pain with inflammation which is aggravated by movement and relieved by moderate pressure and rest. There is inflammation of joints which are hot and swollen. 

Rhus- Tox: 

 Recurring attacks due to getting chilled. Rheumatism caused by damp weather and worse in a damp climate, there is restlessness and the pains are worse on the first motion, better by continued motion. 

Kali-Carb:

  Pain stitching, stabbing, and burning character relieved temporarily by cold application and not by rest or motion. The patient shrieks on account of pain. A backache accompanied by great weakness. 

Arnica

 Chronic arthritis with a feeling of bruised soreness can indicate a need for this remedy pain is worse from touch and may occur in joints that were injured in the past. 

Ruta Graveolens: 

 Arthritis with a feeling of great stiffness and lameness, worse from cold and damp, and worse from exertion often is relieved by this remedy. Tendons and the capsules of the joints may be affected. Arthritis may have developed after overuse, from repeated wear and tear. 

Kalima Latiflora

 Intense arthritis pain that appears quite suddenly may indicate this medicine- especially when the problems start in higher joints and extend to lower ones. Pain and inflammation often start in the shoulder, moving to the elbow, wrist, and hand. The knees are also frequently affected, Discomfort is worse from motion and often worse at night. 

Apis-Mellifica

 This remedy can be helpful in acute conditions with redness, tenderness, and swelling. Joints feel hot and have stinging pain. The hands and knees are often affected. Warmth can aggravate the symptoms and cool applications bring relief. 

Calcarea- Fluorica:

  This remedy can be helpful in acute conditions with redness, tenderness and swelling joints feel hot and have stinging pain. The hands and knees are often affected. Warmth can be aggravated the symptoms and cool applications bring relief. 

Calcarea- Carbonica

 This remedy is often for arthritis in a person who is flabby or overweight and easily tired by exertion. Inflammation and soreness are worse from cold and dampness, and weakness or cramping in the extremities are often seen. Problems often focus on the knees when Calcarea is needed. 

Sticta Pulmonaria:

 Rheumatism of right shoulder blade or joint, ankle joint and knee joint. It also reduces fluid in the joints. 

Belladona

 Acute inflammation of joints marked redness and heat over the affected joint. Congestive constrictive pain in joints. Excessive restlessness with incessant talking, joint pains with spasms of muscles. Worse by heat, checked sweat, pressure, touch movement, better by a light covering, rest in bed. 

Rhododendron

  Fibrous tissues are affected, small joints of the hand are principally affected, also those of forearm and lower limbs, extreme sensitivity to windy stormy and wet weather. Rapidly changing pains, zigzag variety, Descending pains, confused stupid feeling prevails. Affections of wrists, worse before Strome, rough windy weather, night, rest, better by heat, in the sun, motion, wrapping up. 

Pulsatilla

 Venous constitutions, swollen joints with dull aching, associated with numbness of extremities, acute periostitis. Pain in the limbs alternate sides, with the heaviness of legs. Sticking pains in tibia with heaviness and cold sweat on legs, associated with foul foot sweat, chilly patient, yet averse to heat in any form. Associated with digestive disturbances worse by warmth, rest, beginning motion, cold, open air, after a good cry.
