ECZEMA AND HOMEOPATHY
Definition:
It is a type of itching dermatosis characterized by pruritus and chronicity due to antigen-antibody reaction manifesting on the skin associated with Intra and intercellular oedema. The word "Eczema" means flowing over.
Common inflammatory changes like heat, redness, swelling, pain and loss of function may be seen.
Aetiology:
Contact with various agents may give rise to eczema. These are either primary irritants, e.g., acid, alkalies or may be secondary allergens, e.g, Penicillin, streptomycin, lipstick, Nail varnish, Hair dyes, Plants or flowers. Impetigo or ringworm may be complicated by eczematous changes. other factors like genetic, dietetic(egg and milk) and exacerbating factors(heat, humidity, dryness) also play some role. Xerosis-icthyosis, hyperlinear palms, fissures under ear lobe, facial pallor with infraorbital darkening, repeated skin infection, repeated non-specific hand dermatitis are prone to develop eczema. In atopic eczema serum, IgE level is raised. When asthma is present in addition this level is further elevated.
Clinical features:
Adult eczema:
During the acute stage, there is erythema which is soon followed by vesicles which are intensely itching in type. The flexors of the neck, elbow, wrist, knee, ankle are commonly affected. Gradually, the exudates are dried up and crusts are formed. Secondary infection may develop. From time to time fresh exacerbations may occur when oozing may take place, from these patches. This is the subcaste stage. Gradually the skin will be thickened and pigmented. This is called the chronic stage.
Contact eczema:
It is very common and the causes are mentioned above.
Napkin rash:
This occurs on the napkin area of the infants and the irritant is possibly the urine and feces.
Flexural or childhood eczema:
Skinfolds of different parts of the body are affected in older children.
Infantile eczema(Atopic eczema)
This occurs in infants who have an allergy to foreign proteins and are sufferers of asthma, hay fever etc. Facial skin is first affected with erythema, weeping or scale formation. There may be eczematous patches on the flexor aspect of the limbs called Besnier's prurigo.
Dyshidrotic eczema(Pompholyx):
Tapioca vesicles 1-2 mm in diameter may develop on palms and soles as well as dies of fingers and toes. Multiloculated blisters are formed after coalescing of smaller ones. After the blisters are dried upscaling or fissing may develop. This may recur lifelong.
Pompholyx:
This is a type of eczema the sides of fingers, toes, palms and soles irritant vesicle. Bigger vehicles may also develop. History of atrophy is usually absent.
This is commonly seen in young age groups.
Numular Eczema (Discoid eczema):
This is commonly seen in aged individuals. Lower limbs are commonly affected particularly in association with venous engorgement. Rounded or coin like papular lesions are seen.
Eczema Herpetecum:
There is generalized herpes simplex infection with monomorphic vesicles, crusts or erosions on the background of atopic dermatitis.
EXOGENOUS ECZEMA:
There is usually a cell-mediated immune response.
This is of two types:
a) Primary Exogenous eczema, and
b) Contact dermatitis.
In the former type crease of the skin is seen to be affected. Housewife and industrial workers are at risk as they come in contact with the various allergen in the later type cell-mediated immune response (Type IV) is seen. the subject remains sensitive all throughout life.
A patch test can be done to find the offending agent on the back of the trunk.
Homeopathic therapeutics:
Arsenicum Album:
Skin has a white pasty look, with black eruption. Burning and itching, followed by pain after scratching. The sometime offensive discharge which is hot and burning, but relieved by hot application. Eczema worst on uncovered parts or areas. Chilly, weakness. Sips cold water.Restless. Anxious, worried about health, death, security, picky, miserly. Worse: after midnight, cold air or bath, wet; better: hot bathing, warm room.
Arsenicum Iodatum:
There is marked exfoliation of skin in large scales, leaving a raw exuding surface beneath. skin is dry, scaly and itching. Eczema of the beard, watery, oozing itching & worse by washing.
Calcarea-sulph:
There are numerous skin symptoms in Calc-sulph. Unhealthy skin, cuts, do not heal quickly. Burning and itching of the skin. Dryness of skin Itching in bed and better by scratching.
Cicuta Virosa: Marvellous remedy for skin affections.
Eczema: Eczema with no itching. There is exudation from eczema which forms into a hard, lemon-colored crust.
Croton Tiglium:
Eczema all over the body, especially of the scalp and genital organs, either purely vesicular or pustular. The vesicles dry up and are followed by red, raw, inflamed surface. Intense itching of skin, but it is so tender that the patient is unable to scratch, instead rubs gently to get relief.
Graphites:
There is itching of skin over the body with or without an eruption. skin affections are particularly located in the bend of joints, on the scalp and behind the ears. All the skin eruptions have one thing in common, viz, they EXUDE a watery, transparent, gelatinous. sticky fluid. hard cicatrices appearing after mammary abscess, retarding the flow of milk.
The nails are brittle, crumbling and deformed. they are very painful and sore as if ulcerated, thick and crippled.
Clarke says, " The skin of graphites is rough, hard and dry". Eczematous and herpetic eruptions predominate. Eruption on occiput exuding gluey humor, eczema of ears, moist eczema round anus.
Thickened skin. Deep cracks in fingers, heels, etc. nails thick. Grooved.

Hepar Sulphur:
Hepar sulphur is a great remedy for the skin. Every little injury suppurates. The skin is very much sensitive and sore. Eczema spreads by means of new pimples appearing just above the old ones. splinter like pain.
Mezereum:
Eczema and itching eruptions after vaccination, with intolerable itching followed by copious serous exudation oozing out aggravated in bed and from touch.
The favourite site for many of the eruptions is the scalp.
Petroleum:
The skin of petroleum is rough, ragged, exfoliated and bleeding: Cracks and fissures all over the body" or in affected area, is the keynote symptom of petroleum.
Eczema especially of scalp and occiput.
Psorinum:
Eczema due to suppression of skin eruption. Skin looks dirty and unhealthy even after washing. The eruption may be dry, scaly or moist. Whatever may be an eruption of the skin, it becomes thick and indurated. The skin is very much sensitive to atmospheric changes. There is intense itching which leads the patients to despair. The eczema of scalp, face and behind the ear oozes out WATERY AND FOETID DISCHARGE. The skin eruption is worse from bathing and warmth of bed.
Sulphur:
Skin affections that have been treated with medicated soap and washes, suppressed by ointment and local medication. Dirty, filthy look of the skin, with filthy odor. The skin is very much sensitive to atmospheric changes. The patient does not like to be washed or bathed. There is severe itching, voluptuous itching; goes on scratching.
